The size of the milieu in PHP varies wildly, when I was there it was huge. Like 30 to 35 people.

Medical every other week, psych once weekly, therapist 2x weekly, RD 2x weekly, nursing once a week or as needed.
There were like 2-3 BHS/BHTs for the entire milieu at any given time. Patients were pretty much “free roaming” and could come and go from the group room as they pleased.
Mostly DBT. Some CBT and some ACT values-work type stuff.
Describe the average day:
8:00am: Arrival and sign-in. Almost everyone is normally late haha.
8:30am: Breakfast.
9:30am: Group.
10:30am: AM snack.
11:00am: Downtime.
11:30am: Group.
12:00pm: Group.
12:30pm: Lunch.
1:30pm: Group.
2:30pm: Dismissal.
(Dismissal is at 1:45pm on Saturdays).
Yes, less than 50% was two Boost plus and 50-99% was one Boost plus. They also had Boost breeze and Kate Farms but both of those required RD approval.
Yes to both as far as I’m aware.
DBT, CBT, yoga, boundaries, relapse prevention (they call it “embracing recovery”), psychoeducation, structured discussion, community, art group on Saturdays.
I did res first so tbh the groups did get pretty redundant for me after a while because it’s basically the same stuff. I did like the art group on Saturdays though!
Nope. Have a history of complex trauma and I’m pretty confident the program didn’t know what to do with me bc of it.
I REALLY liked the team I was assigned. Even though they didn’t quite know what to do with me they did their best to work with me despite the circumstances and helped make things somewhat bearable. I would’ve discharged much earlier than I did if not for them.
There are also a couple of staff (BHS) who are absolutely wonderful and also made my time there more bearable.
A lot of themes in programming were redundant or not applicable to me because of my history/co-occuring mh conditions. They sort of push the same cookie cutter version of recovery on everyone and they do not know how to treat co-occurring PTSD. The fact that the milieu was so big was DEFINITELY an issue too. There really weren’t enough staff to keep eyes on 35 people, [TW] I got away with a lot of behaviors because of this [TW over]. I know this has been somewhat resolved because they’ve since split the milieu in half but still, I feel like 30-35 people and two to three staff is an INSANE ratio that never should’ve been allowed to happen in the first place. The staff they did have seemed visibly stressed and stretched thin and I always felt bad about bothering them even if it was because of something serious because I knew they already had 1000 things to do. It felt like the program was just trying to admit as many people as possible, it made me feel like a statistic, like I was only there so they could bill my insurance :/.
Not really honestly. I stepped down from res and even though I liked the team I was assigned in PHP there was just so much else wrong with it. I ultimately feel like it did more harm than good.
They have a yoga group like 2-3x a week. I think that people’s activity levels outside programming probably varied depending on their team’s recommendations.
There wasn’t programming on Sundays, but Saturdays were normally really boring. A lot of people had passes on Saturdays so there was almost no one there most of the time.

In PHP at least half the milieu are normally already weight restored + have stepped down from res. I think if not weight restored they expect like 2lbs/week? They’re super vague w that information though.
Lengths of stay varied wildly. Probably between 8-14 weeks. I knew a couple people who were only in PHP for a month or so before discharging, and a couple who had been in PHP for 3+ months.
18-50, the age range varies more in PHP than it does in res imo.
They have an electronics policy, but it wasn’t really ever enforced when I was there. I think this may have changed though.
Not a whole lot. You can message your team on recovery record or email them but if it’s outside program hours you might not hear back super quickly.
I already had an outpatient team but they will help you set one up/send out referrals if you don’t already have one.
They have housing for PHP if you’re coming from out of state but it’s very limited from my understanding. I knew a couple people who were from out of state and had to stay in hotels or Airbnbs long term because they couldn’t get into the housing.

The Richmond location has closed. In all the years it was open they never seemed to be able to get above a few patients at a time. I don’t know if they will reopen. In the meantime, Aster Springs is new residential in Richmond that also has a PHP (not sure on IOP) that also includes housing, so if you are local to RVA, that may be an option.
I did their virtual php from their durham location Summer 2021 – it was okay. groups were incredibly boring and you sat in silence during meals because the real conversation was happening in person. definitely a weird set up, but i had an amazing team so it made up for it.

How many patients are there on average?
In PHP at one point there were only 4 of us. At another point it got up to 10. When I was in IOP there were only 4 or 5.
Does it treat both males and females? If so, is treatment separate or combined?
Yes, combined.
How often do you see a medical doctor, psychiatrist, psychologist (therapist), nutritionist, etc?
PHP- Medical doctor when you admit and then as needed. Psychiatrist a couple of times a week for check ins. Therapist and dietician twice a week. In IOP you can choose to go back to your outpatient team or see the Veritas team once a week but you have to come in outside of programming for those appointments, which was stupid.
What is the staff-to-patient ratio?
One or two TA’s depending on the number of patients. One nurse.
What sort of therapies are used (e.g., DBT, CBT, EMDR, etc.)?
Lots of DBT. It’s super repetitive.
Describe the average day:
PHP:
7:30-8:00- arrival, weights, vitals
8:00-9:00- breakfast
9:00-9:30- free time
9:30-12:30- groups (morning snack 10:00 if you have it)
12:30-1:30- lunch
1:30-2:00- meal processing
2:00-3:00- group
3:00-3:30- snack
3:30- leave
IOP (Monday, Wednesday, Thursday)
4:45-5:45- group
5:45-6:45- dinner
6:45-7:00- meal processing
7:00-8:00- group
What were meals like?
For PHP, you would got to the IP/Res unit kitchen and plate your breakfast. Whatever exchanges you had. Lunch was usually served family style and brought to the unit, but some days you would go to IP/Res and plate. Snacks you played yourself on the unit based on exchanges. For IOP, dinner was brought to the unit on Monday and Thursdays. On Wednesday you brought your own dinner. You could bring takeout or something from home.
What sorts of food were available or served?
Pretty typical. Breakfast they had scrambled eggs, bacon, toast, English muffins, cereal, yogurt. I hated their breakfasts. Lunch they always had like chicken and rice things. Sometimes pasta, chili, hot dogs, weird sandwiches. It was a 4 week rotation for lunch and dinners.
Did they supplement? How did that system work?
Yes, one boost if you finish half, two for less than half. Or you can have a Veritas shake, which is like a strawberry milkshake.
What is the policy of not complying with meals?
I got stepped back up to res. They will make a contract.
Are you able to eat vegetarian?
Yes.
What privileges are allowed?
None on PHP/IOP. Meaning same rules for everybody.
Does it work on a level system?
Not php/IOP.
How do you earn privileges?
N/a
What sort of groups do they have?
Boring groups. Interpersonal process group, chain analysis, yoga, art, nursing/med education group, DBT, vocational group, culinary, CBT, body acceptance, RO-DBT, cope ahead, relapse prevention.
What was your favorite group?
I hated them all to be honest.
What did you like the most?
Playing games during free time.
What did you like the least?
Everything. Staff at Veritas is inexperienced and not helpful. Groups are super boring and repetitive. It’s like pulling teeth to skip a day of PHP. My contact fell out one day and I couldn’t see and they would not let me leave.
Would you recommend this program?
NO!
What level of activity or exercise was allowed?
Whatever in PHP/IOP.
What did people do on weekends?
Go to PHP. Schedule is a little different but we still had it.

I don’t know. I think everybody is already weight restored from IP/res but idk.

You have to turn in your phone when you get there.

You can’t have any of that.

You eventually get to start taking days off as you move on in the program.

Yes.

So I definitely wouldn’t recommend Carolina House. Between the two, Veritas is better. My therapist said that Veritas has gotten a lot better over the last year or so. Also, they have just merged with the Emily Program, which is apparently helping things. I know that they have somebody who has been coming in and training the therapists. As much hate as I talk about Veritas, I may be going back myself (for IP/res) because yeah, there just aren’t that many great programs in NC. The only other thing I know of is Tapestry, but I haven’t been and don’t know much about it.

I’m looking at the leadership team page on Veritas’ website, and most of the senior leadership list is now working at both Veritas and Emily Program. I wonder if this is Emily Program’s expansion into being able to offer inpatient/acute residential at an affiliated facility? Lack of inpatient/acute has always been one of TEP’s main struggles, especially the last few years as places across the country become more vertically integrated. They always have to refer out for IP – Melrose Center, Sanford EDI, ERC, Belmont, Brandywine, Children’s, etc – and I’m sure they lose a lot of patients that way.
–Obviously these are just my initial thoughts, based on what I’m seeing so far, so take them with a ten packets of salt and don’t quote me lol!
Thank you so much for sharing!! I was between Carolina House and Veritas so this is super helpful. I have not been to treatment before, so I don’t know what all I need to look for. Is there anything I should specifically ask Veritas? I have heard that they can try to get a lot of money out of you by having you stay longer or do higher levels of care than you need. Did you experience this at all? My goal is to do PHP or IOP, not IP/res, so I don’t want to be forced to do IP if I don’t really need it.
Hopefully things are better now though because of all the changes!

I was at the Durham location! In my experience, it is kind of difficult to get insurance to pay for treatment as it is, so it’s hard for me to imagine getting insurance to pay for a higher level of care than you actually need. I don’t know of anybody who has experienced this. For me, I always transferred to Veritas from an out of state facility so I never experienced this. They did step me up from PHP back to res last time I was there, but I was restricting a lot and not completing most meals and snacks so it made sense. Also, they usually make a contract with you if you are in danger of needing to step up so you know what you need to do to stay at your current level of care. Lastly, you will do a phone assessment with them where they will determine which level of care you need so based on that they will decide! And they will ask you what level of care you are looking to do. But I know a lot of people who started at the PHP and even IOP level. Let me know if you have any other questions!
Veritas says you need a higher level of care so insurance will pay for it as they are relying on that info.
My own (ED specialist) therapist thinks that Veritas tries to get everyone into the highest level of care that they can. It seems if you have any behaviors they want you IP.
Sorry, Veritas has been the very worst treatment facility I have ever been to and find their business practices to be sketchy.

Yeah, I just received my records from Veritas. In it they state that I struggled with “intense suicidal ideation” for the majority of my stay. I never once indicated I was dealing with suicidal thoughts, and every time they asked (which was daily), I always said no. I feel like they made these claims because they wanted to hold me in a higher level of care.
In my notes for PHP they said I “frequently” did not show up for care. I missed two days in 8 weeks. One was because I wasn’t feeling well, and one was because I live in the center of the city where a hug marathon event was taking place and would have had to walk 3 miles to get to a car. They said I didn’t try hard enough.
There were a lot of other negative things too about my participation and things like that that I felt were NOT accurate at all. It was disturbing.
Fifteen years ago when I went to treatment, getting most insurance companies to cover more than 10 days of inpatient eating disorder treatment if you weren’t medically unstable or you weren’t suicidal, or if your treatment facility didn’t fudge reality when speaking with your insurance and claim you were, was extremely difficult, and there were maybe 5 residential treatment centers in the country, and it was even harder to get insurance to cover those. I can think of dozens of people, maybe more than 50?, whose lives would be completely different today if they had been able to stay at a higher level of care for more than 3 weeks. Some of whom in the sense that they would actually have the opportunity to enjoy their lives because they wouldn’t have ended up with chronic EDs because they were well past the 5-10 year mark of illness by the time insurance companies began to change, and then residential treatment became normalized and most states or at least regions had a residential option. Some of whom in the sense that, they passed away, so they would actually just still be alive.
Hopefully the future of eating disorder treatment centers and insurance coverage will find a balance between the way things were then, and the things some of you are experiencing now on the other end of the spectrum.
